Appointment reminders,
illnesses & safety protocol
Reminders: You will receive a reminder confirmation call the day before your scheduled appointment. if you should need to cancel your appointment, please see below for our cancellation policy.
illnesses: If you have flu or cold symptoms to include: fever, cough runny nose, or have been exposed to anyone with these symptoms in the past 72 hours, you must inform your phlebotomist prior to arrival. You will be required to wear a mask for safety precautions.
Drawing children:
Tip: have distractions available i.e.: toys, favorite stuffed animal, etc. talking to your child prior to the draw with positive encouragement is helpful. For the safety of the patient and the phlebotomist, if the child becomes un-manageable, the lab draw will not be performed, and you will not be charged. Thank you for your understanding.
ORDERS:
You must have a lab requisition from your ordering physician.
A copy of your insurance card and ID will be required for specimen drop off.
Your information will not be shared or used for any purposes other than for lab billing purposes only.
Explanation of fee's
Fee's are charged as convenience services.

Mileage fee will be added for overages
of 10 miles.
The fee's are determined from the patients home address to the nearest drop off location.
(most drop off locations are within a few miles however if there should be a special request and it is over 10 miles then the fee of $5.00 would apply)
your lab tests will be directly billed to your insurance by the laboratory.

Cancellation Policy
Appointments must be cancelled within 24 hours prior to your visit.
If your appointment is not cancelled within the allotted time frame, a $50 non-cancellation fee will be applied.
Refund Policy
There are no cash refunds. If your test needs to be repeated for any
reason, we will come out to re-collect your specimen at no charge.
